We still consult but anything we do is pro bono. if i do not charge for something, i do not have put up with getting approval from an insurer or write what they want (even if it is totally not indicated or the best medication). I am fortunate having practiced mostly before all of this control and regulation.
    I ask them for their full name and credentials as I will be documenting in the chart their refusal to OK a treatment I, physically seeing the patient, have deemed necessary. That gets me to the supervisor almost every time. They won't give me what I need but I get their name.
    I wish most of my patients only want to do what will cost them nothing- many of mine feel they are entitled to get what ever they want, when they want it because they should get what they want. Period. More and more of that. Couple that with a PHO that is ridiculously obstructionist and unashamedly all about the money and I have had enough. When the PHO refuses a referral for someone to see the Doc they have had for 25+ yrs and doesn't even sugar coat the reasoning.... Everything is about $$$. The PHO sold their souls for the almighty $ to most of the insurance co. The Specialists outnumber the PCPs so they make out fairly well. The PCPs are getting slammed. I know of 3 Docs getting out and more than a few NP/PAs. One National survey said 63% of Primary providers described themselves as burnt out and very dissatisfied. I would say that is about right. The new Docs coming out seem to think they should have business hours and no hols or WE> The new NPs coming out seemed to be trained to be martyrs like Joan of Arc. I am way too cynical to have the patience for it. (I obviously am one of those 63%.)

    Yep- I got married and had a kid but lost most of my social contacts, have a wrecked bod and feel the same way. I have a gift for what I do but they won't let me do it the right way anymore. At some point I will probably find some volunteer thing to feed the need.

                                Great man for college hockey. At the 2000 Final Four(I refuse to use that marketing term), he made a mayoral proclamation that within something like a four block radius of the Providence Civic Center, you could carry an open container of alcohol. Second best Final Four I've ever been to. What a great weekend.
