Haven't watched since Monday, but from what I've read, Helena has been keeping Stavros on ice since his death. She had Lucky poison Lucas so that Tony would work to bring him back to life in exchange for her saving Lucas. I think today his heart starts beating. The actor playing him is younger than Stefan, his younger brother. I guess when you're on ice you don't age. Though I think he could pass for Nik's dad. Helena is one twisted sister.
